How to Reduce No-Shows at a Med Spa

An empty treatment room still costs you the provider’s time, the room and the slot someone else could have booked. Most no-shows aren’t people who never meant to come. They forgot, their plans changed, or rescheduling felt like more effort than not showing up. Each of those has a fix.

1. Take a deposit or a card at booking

A deposit is the strongest signal that a booking is real. For consultations, many med spas take a small deposit that comes off the treatment price. For longer treatments, a card on file with a clear no-show fee works too. Either way, say exactly what happens if someone misses the appointment, at the moment they book.

2. Write a cancellation policy people can actually find

Pick a notice period, usually 24 or 48 hours, and a fee. Then show the policy in three places: on the booking page, in the confirmation message and in the reminder. A policy that only lives in the small print doesn’t change behavior. One that people read twice before the appointment does.

Example: Need to change your appointment? Reschedule or cancel at least 24 hours before and your deposit carries over. Missed appointments or later changes forfeit the deposit.

3. Send reminders by text, at the right times

Text reminders work. A Cochrane review of healthcare appointments found that attendance rose from 67.8% with no reminder to 78.6% with text reminders, about the same effect as phone call reminders at a lower cost (Cochrane, 2013). A schedule that suits most med spas:

  1. At booking: a confirmation with the date, time, address, what to bring or avoid beforehand, and the cancellation policy.
  2. Two days before: a reminder with a reschedule link. This is early enough for you to refill the slot if they can’t make it.
  3. The morning of, or two hours before: a short final reminder, with parking or arrival tips if they’re useful.

Keep reminders free of treatment details. “Your appointment at Glow Med Spa is tomorrow at 2:00” is enough.

4. Ask for a reply and make rescheduling one tap

Ask people to reply C to confirm. Unconfirmed appointments become your call list for the morning. Then add a reschedule link to every reminder. Someone who can’t make it will move their appointment if it takes ten seconds. If it means calling during opening hours, many will simply not turn up.

5. Book new leads soon

The longer the wait between booking and the appointment, the more time there is for plans and enthusiasm to change. This matters most for new leads from ads. Offer consultation times within the next few days, and save the far-off dates for regulars who already know you.

6. Keep a short waitlist

When someone reschedules two days ahead, a waitlist turns that gap into a booking. Note clients who wanted an earlier time, and text them when a slot opens. First to reply gets it.

7. Follow up the same day after a no-show

A friendly message the same afternoon brings many no-shows back. Keep it warm, not accusatory, and include the booking link.

8. Measure your no-show rate

Track no-shows each month, split by treatment and by where the client came from. If consultations from one ad campaign miss far more often than referrals, fix that campaign’s follow-up first. If one treatment stands out, look at how far ahead it’s usually booked.
